We attempt to minimize  medications and excess or unnecessary tests and focus on lifestyle changes,  diet, exercise, and other modalities to help you to realize your best  health.


NEW PATIENTS:
1. Please call to make appointment for physical, check-up, or symptom(s): 212.537.5039
    -when making appointment ask for user id and password. 
2. Log on tho the "Patient Portal".    
3.  In the "Patient Portal", please fill out your personal information   and "Web Interview"
      Please fill out  prior to appointment. (at least one day prior)
     In the web interview, select the option that best fits your reason for the appointment
      -for example for a physical or check up, select "adult routine medical exam", then answer questions
      -for example if having chest pain, select "chest pain", then answer questions


ESTABLISHED PATIENTS
1.  To receive a user id and password for the patient portal,  please call the office (212-537-5039) and ask for user id and password.
2.  Log into the Patient portal above.
3.  In the Patient Portal,  update your phone number, and address under "patient information"
4.  In the patient portal fill out "web interview" to get the most out of your visit.
     In the web interview, select the option that best fits your reason for the appointment
      -for example for a physical or check up, select "adult routine medical exam"
      -for example if having chest pain, select "chest pain"
